Immediate Full-Arch Rehabilitation of the Mandible

Procedure:

Immediate extraction, immediate implant placement, and immediate loading – mandibular full-arch prosthesis

Clinical Scenario:

A patient presented with generalized mobility of mandibular teeth associated with multiple infected sites. The case required a rapid, predictable full-arch solution with functional and esthetic rehabilitation in a severely compromised mandibular arch.

Treatment Protocol:

The entire procedure was completed under local anesthesia and followed a start-to-finish immediate loading protocol over 3 days.

Full Mouth Extraction (Mandible):

All remaining mandibular teeth were atraumatically extracted.

Infection Management:

Thorough debridement of all infected and granulation tissue was performed to ensure a clean and stable surgical bed.

Implant Placement:

Free-hand placement of implants was carried out in the mandibular arch, achieving optimal primary stability despite compromised bone conditions.

Prosthetic Phase:

Placement of multi-unit abutments

Intraoral digital impression recorded using iTero Lumina

Fully digital prosthetic workflow employed for prosthesis fabrication

Immediate Loading:

A definitive full-arch prosthesis was delivered within 72 hours (3 days) of surgery, restoring function and esthetics immediately.

Outcome:

The patient received a stable, functional, and esthetically driven mandibular full-arch prosthesis within 3 days, demonstrating the effectiveness of immediate extraction, immediate placement, and immediate loading protocols even in the presence of infection—when executed with meticulous surgical and prosthetic planning.

Key Highlights:

Immediate extraction, placement, and loading

Management of multiple infected sites

Free-hand implant placement

Complete digital workflow

Final prosthesis delivered in 72 hours
