Case Summary 🔎

Patient Information

  • Age / Sex: 50-year-old female

  • Medical History: No relevant systemic disease

  • Chief Complaint: “My overall dental condition is poor.”

  • Clinical Diagnosis: Generalized severe periodontitis affecting the remaining maxillary and mandibular teeth

Case Summary

All remaining maxillary teeth were extracted, followed by surgical guide-assisted placement of six implants. Multi-unit abutments were connected, and a 3D-printed resin provisional prosthesis was delivered on the day of surgery.

In the mandible, the existing implants were maintained, the remaining periodontally compromised teeth were extracted, and one additional implant was placed in the mandibular anterior region. Full-arch rehabilitation was completed with a maxillary All-on-6 and mandibular All-on-5 prosthesis.

Case Focus: Guided surgery, All on X, Full Arch, MUA connection, and same-day provisionalization.

Case Presentation

1️⃣Pre-op

  • Pre o.p panorama

  • Preoperative intraoral and smile photographs

  • Preoperative intraoral scan data and provisional prosthesis design for immediate provisionalization using exocad.

2️⃣ Maxillary Surgery

  • Post-extraction intraoral view and surgical guide placement.

  • Connection of multiunit abutment

  • #16: ALX-IT Ø5.0 × 10 mm, cuff 3 mm, D332, 40 Ncm

  • #14: ALX-IT Ø4.5 × 12 mm, cuff 4 mm, D032, 30 Ncm

  • #11: ALX-IT Ø4.5 × 10 mm, cuff 4 mm, D332, 40 Ncm

  • #21: ALX-IT Ø4.5 × 10 mm, cuff 4 mm, D332, 40 Ncm

  • #24: ALX-IT Ø4.5 × 10 mm, cuff 3 mm, D002, 40 Ncm

  • #26: ALX-IT Ø5.5 × 10 mm, cuff 3 mm, D332, 40 Ncm

  • Connection of the Scan Bars on the Scan Posts

3️⃣Maxillary Prosthetic

  • Connection of the scan bar after attaching the Scan Body Kit scan posts.

  • Implant position scan data were merged with the soft-tissue scan data to accurately transfer the implant positions and surrounding soft-tissue contours

  •  Virtual Mount of the Maxillary Implant IOS Model

  • Complete the Provisional Design on the implants

  •  Print the provisional in the dental clinic

  •  Provisional Delivered in 1 Hour After Surgery

  • 3 weeks after surgery

4️⃣ Mandibular Surgery & Pros

  • Mandibular scan post data and postoperative panoramic radiographic evaluation.

  • Following extraction of teeth #41–34, a surgical guide was used to place an additional implant at the #32 site, while the existing mandibular implants were maintained.

  • #32: ALX-BT Ø4.5 × 10 mm, D221, 40 Ncm

  • Connection of multi-unit abutments and scan posts after removal of the existing mandibular prosthesis.

  • Mandibular scan post data and postoperative panoramic radiographic evaluation.

  • Connection of the scan bars to the scan posts, followed by acquisition of the mandibular implant scan data.

  • Merging the postoperative implant scan data with the preoperative prosthesis design for provisional prosthesis fabrication.

  • Same-day delivery of the 3D-printed provisional prostheses, followed by intraoral occlusal and extraoral smile evaluation.

  • Post o.p panorama

  • Extraoral and intraoral views at the 1-month follow-up after provisional prosthesis removal, showing soft-tissue healing around the maxillary and mandibular multi-unit abutments.

5️⃣ Definitive Prosthesis Delivery

  • Definitive Prostheses in 3 months

  • Superimposition of the provisional prosthesis data and definitive prosthesis design.

  • Fabricated maxillary and mandibular definitive prostheses

  • 3-month follow-up prior to definitive prosthesis delivery

  • Seating and screw fixation of the maxillary and mandibular screw-retained definitive prostheses.

  • Before & After
