Early Loading of ALX-BT Implants in Different Bone Densities

Patient Information
Case Summary 🔎
A 64-year-old healthy female presented with healed edentulous sites at #36 and #37 following extraction in March 2026. ALX-BT implants (Ø4.5 neck / Ø5.0 body × 10 mm) were placed using an undersize drilling protocol. The bone densities were classified as D333 at #36 and D223 at #37. Despite the differences in bone density, adequate implant stability was achieved, allowing early loading with a splinted full-zirconia crown 3 weeks after implant placement.
Case Presentation
1️⃣Pre-op

2️⃣Surgery
#46 healed edentulous site, with mild concavity & limited keratinized tissue of buccal gingival tissue.
After full flap operation


CMI Bone densities & Drilling bur steps:
- #36 (D333): D2.2-D3.0-D3.5 with L11.5mm
- #37 (D223): D2.2-D3.0-D3.5 with L11.5mm & D4.2/L10mm (final drill)
Packing moldable Xenograft into the socket defect and performing buccal contour augmentation.


3️⃣Prosthesis

CAD-CAM fabrication of Splinted full-Zirconia crown (#36, #37implants).

After #36, #37 implant placement 3weeks, IST remeasurement. #36 IST value = 73(+4) & #37 IST value = 83(+2).

Final prosthetic delivery.


4️⃣Follow up
3-week follow-up after final restoration delivery.
